What is Rybelsus?
Rybelsus tablets (semaglutide) are used for adults with type 2 diabetes to control blood sugar levels and reduce A1C. Rybelsus is also used to reduce the risk of major cardiovascular events such as heart attack, stroke, or death in adults with type 2 diabetes who are at high risk for these events.
Rybelsus has been shown in clinical studies to assist with weight loss; however, it is not FDA-approved as a weight loss medicine. It is taken as a once-daily tablet alongside diet and exercise.
Rybelsus contains the same active ingredient — semaglutide — as Ozempic (injection) and Wegovy (injection). Wegovy is FDA-approved for weight management and the serious liver condition MASH; Ozempic is approved for type 2 diabetes and related conditions.
Important: Rybelsus (R1 formulation) and the Ozempic oral pill (R2 formulation) are not interchangeable on a dose-per-dose basis because they differ in inactive ingredients that affect how the drug is absorbed. There is currently no generic Rybelsus available. This medicine is not for type 1 diabetes.

Uses & Indications
- Type 2 Diabetes Mellitus: Improves blood sugar control and reduces A1C as an adjunct to diet and exercise.
- Cardiovascular Risk Reduction: Reduces the risk of major adverse cardiovascular events (MACE) — heart attack, stroke, or cardiovascular death — in adults with type 2 diabetes at high cardiovascular risk.
- Off-label Weight Loss: Clinical trials show meaningful weight loss (see FAQ); not FDA-approved for this indication.
Rybelsus is not approved for use in people under 18 years of age or for the treatment of type 1 diabetes.
How Rybelsus Works
Rybelsus belongs to the class of medicines called GLP-1 receptor agonists (glucagon-like peptide-1 receptor agonists). It lowers blood glucose through three complementary mechanisms:
- Increases insulin secretion in a glucose-dependent manner
- Slows gastric emptying, reducing post-meal glucose spikes
- Decreases glucagon secretion, lowering hepatic glucose output

Common Side Effects
- Low blood sugar (hypoglycaemia) in people with type 2 diabetes
- Upset stomach, heartburn, burping, gas, bloating
- Nausea, vomiting, stomach pain, loss of appetite
- Diarrhoea or constipation
- Runny nose or sore throat
- Stomach flu symptoms
- Headache, dizziness, tiredness

⚠ Emergency: Allergic Reaction
Get emergency help immediately if you experience hives, itching, dizziness, fast heartbeat, difficulty breathing, or swelling of the face, lips, tongue, or throat.
Call your doctor at once if you experience any of the following:
- Vision changes
- Unusual mood changes or thoughts of self-harm
- Pounding or fluttering heartbeats
- Light-headedness or near-fainting
- Thyroid tumour signs: neck swelling or lump, trouble swallowing, hoarse voice, shortness of breath
- Pancreatitis: severe upper stomach pain radiating to the back, nausea/vomiting, fast heart rate
- Gallbladder problems: upper stomach pain, fever, clay-coloured stools, jaundice
- Low blood sugar: headache, hunger, weakness, sweating, confusion, irritability, dizziness, fast heart rate, jitteriness
- Kidney problems: swelling, reduced urination, fatigue, shortness of breath
- Stomach flu: cramps, vomiting, loss of appetite, watery or bloody diarrhoea

Surgery / Anaesthesia Alert: Pulmonary aspiration during general anaesthesia or deep sedation has been reported in patients receiving GLP-1 receptor agonists. Always inform your surgical team that you are taking Rybelsus before any planned procedure.
Warnings & Precautions
⚠ BOXED WARNING – Thyroid C-Cell Tumours
In laboratory animal studies, semaglutide caused thyroid C-cell tumours in rodents. It is unknown whether this risk applies to humans. Do not use Rybelsus if you have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia type 2 (MEN 2).
Do Not Use Rybelsus If You Have:
- Multiple endocrine neoplasia type 2 (MEN 2)
- Personal or family history of medullary thyroid carcinoma
- Insulin-dependent (type 1) diabetes
- Diabetic ketoacidosis
- Allergy to semaglutide or any ingredient in Rybelsus
Tell Your Doctor If You Have:
- Stomach or intestinal disorder
- Gallbladder disease
- History of pancreatitis
- Planned surgery or procedures requiring anaesthesia or deep sedation
- Diabetic eye disease (retinopathy)
- Kidney disease
Pregnancy & Breastfeeding
Stop Rybelsus at least 2 months before planned pregnancy. Controlling diabetes during pregnancy is essential; consult your doctor for a safer alternative. Even if overweight, losing weight during pregnancy can harm the unborn baby. Do not breastfeed while using this medication.

R1 vs R2 Formulations: A newer formulation (R2) has been approved. Both contain semaglutide but differ in inactive ingredients affecting absorption. They are not interchangeable on a dose-per-dose basis.
How to Take Rybelsus
- Take once daily, first thing in the morning.
- Take on an empty stomach with no more than 4 oz (120 mL) of plain water only.
- Wait at least 30 minutes before eating, drinking anything else, or taking other oral medicines.
- Swallow the tablet whole — do not crush, chew, or break it.
- Do not use different brands of semaglutide at the same time.
Blood sugar can be affected by stress, illness, surgery, exercise, alcohol, or skipped meals. If you experience prolonged vomiting or diarrhoea, contact your doctor as you may become dehydrated. Low blood sugar can be quickly treated with hard candy, fruit juice, crackers, raisins, or non-diet soda.

Missed Dose
Skip the missed dose and take your next dose at the regular time the following morning. Do not double up to make up for a missed dose.
Overdose
Seek emergency medical attention or call the Poison Control line at 1-800-222-1222. Overdose may cause severe nausea, vomiting, or dangerously low blood sugar.
Drug Interactions
Rybelsus can slow digestion, meaning other oral medications you take may be absorbed more slowly. Always tell your doctor and pharmacist about all medicines you take, including:
- Insulin or other diabetes medicines (e.g., dulaglutide, exenatide, liraglutide, Trulicity, Victoza)
- Any prescription or over-the-counter medicines
- Vitamins and herbal supplements
